Application Form for Fellowship in Sport Physiotherapy

The sport Physiotherapy Fellowship is a ten month program (August 1st
- May 31st) that is designed to give the fellow exposure to a wide variety
of sport physiotherapy condition/situations. The curriculum will include
clinical experience, research, teaching, and education.
The objective of this program is to become more versed in the treatment
of athletic injuries, initiate a research interest and be qualified and
prepared to write/sit the S.P.D. Level II examination.
Pre-Requisites
- Member of the Canadian Physiotherapy Association (C.P.A) and the
Sport Physiotherapy Division.
- Eligible for and registered to practise physiotherapy in the province
of Ontario .
- Certified S.P.D Level I
- Registered as a candidate for S.P.D. Level II

Responsibilities
Clinical Experience
- To carry a physiotherapy caseload, five half days per week.
- To cover at least one varsity team from beginning to end of the season
(including pre-season physical, conditioning, practises, games, and
rehab).
- To spend observation time in: orthopaedic clinics, primary care clinic,
orthotics clinic or surgery.
Research
- To review, initiate and complete at least one research project.
- To support and assist with any ongoing research projects.
Education
- To attend rounds and in-services as appropriate to the specialty
of sport physiotherapy (i.e. orthopaedic, research, sport medicine,
physiotherapy).
- To attend Kin Course 436 (theory) and 485 (practical-selectively)
in athletic therapy on an audit basis.
- To attend appropriate post-graduate physiotherapy courses, seminars
and conferences. (Five days of education leave will be provided in
addition to UWO Sport Medicine Symposium and Orthopaedic Residents
Day.)
Teaching
- To assist in the supervision of physical therapy students.
- To assist in the teaching and "shadowing" of the undergraduate
physical education and high school co-op students.
- To be available for physiotherapy consultation and collaboration
with physicians.
- To assist with community involvement and promotion of the specialty
of physiotherapists in sport medicine (i.e. lectures, workshops, career
days).
Administrative
- To act as the "Chief Therapist" for the London Marathon
in association with the "Chief Medical Officer" who is customarily
the primary care sports medicine fellow.
- To assist in coordinating special event coverage as requested.
The Fowler Kennedy Sport Medicine Clinic is offering
a sport physiotherapy fellowship. The purpose
of the fellowship is to enhance and support specialization in the treatment
of sport injuries/conditions.
The curriculum will include: clinical experience, research, teaching,
education, and on-field/event coverage.
